Tidewater Health is required by law to maintain the privacy of your health information as well as provide individuals with notice of its legal duties, privacy practices and your rights with respect to health information. These notices will be provided to you– for your consent and approval– at or prior to your first appointment. Tidewater Health Center is required to abide by the terms of theses notices currently in effect. Tidewater Health reserves the right to change the terms of its notice and to make the new notice provisions effective for all Protected Health Information that it maintains.

How do you get my consent?
All notices and practices will be provided to you–for your review and consent– at or prior to your first appointment.

We may disclose your personal information if we are required by law to do so or if you violate our Terms of Service.

When you provide us with your email address as part of the registration or appointment setting process, we will place you on our list of patients to receive informational and promotional emails. In addition, patients and visitors to our website are given the opportunity to “opt-in” to receive electronic promotional communications by selecting the option to receive promotional emails from us on our website.
You may “opt-out” at any time from our promotional emails by clicking the link to “unsubscribe” or “opt-out,” located in the footer of any e-mail we send.
